Monogram Etiquette

Due to the popularity of the invitation etiquette post I did a few weeks ago, I decided to touch on monogram etiquette. Here are examples of name usage, order and format to be used in monogram design and personalized goods. What do you recommend for a last name comprised of two words, for example Van Pelt?
